Frequently Asked Questions

How does this process actually work?
We connect to your billing system once, then your underpaid claims flow to us automatically. We handle everything: filing, deadlines, arbitration, legal briefs. Insurance pays you directly. You pay our fee only after you get paid. You keep practicing medicine. We recover your money.
What % do you charge, is it only on money you recover?
We charge 20% of what we recover, and ONLY on the additional money put in your pocket. If we don't recover anything, you owe us nothing. Some firms charge less, but we consistently recover more per case than other providers on similar claims. Surgeons often take away more by working with us than they would on a small percentage. The percentage matters less than what actually lands in your account.
Do I pay anything upfront?
Zero. We front all costs: the filing fees, the arbitrator fees, everything. You never write us a check until insurance pays you first.
How long do I have to file a claim?
30 days from the date insurance pays you. Miss it by even one day, and that claim is typically gone. That's why we move fast and track every deadline for you.
Can you recover old claims past the 30-day window?
In most cases, once that 30-day federal window closes, the claim is gone. However, some states have their own laws with longer filing periods (eg. New York allows up to 3 years). We can review your situation and explore whether any state-level options apply. We recommend providers book a free recovery call to start capturing every eligible claim going forward.
What's your win rate?
90%. We've recovered over $1 billion using a process we've refined over 27 years. We know which arbitrators favor which arguments, and we file with precision.
What types of cases qualify?
Out-of-network emergency cases. On-call surgical work. Inadvertent services at in-network facilities. If you treated a patient who didn't choose to go out-of-network, it likely qualifies. Medicare, Medicaid, and TRICARE do not qualify. The insurance company must have made some payment, and the recoverable amount should be $3,000+.
What do we need to do on our end to get started?
Sign two short agreements (a BAA and CFA), then send us three things per claim: the billing form, the EOB, and the operative report. Takes about 10 minutes. All HIPAA compliant. After that, new claims flow to us automatically each month. No ongoing work from your team. You can review the BAA in advance by clicking here.
Is the patient involved or affected in any way?
No. The patient is completely out of the picture. This is between you, the insurance company, and us. The money comes from the carrier, not the patient.
I already have a billing team. Why do I need this?
We don't replace your billing company. We extend them. Most billing companies stop before arbitration because it's complex and time-sensitive. Most of our clients keep their existing billing operations exactly as they are - we're just the recovery piece that gets you additional money they're not set up to chase. No disruption, no conflict.
What happens if we lose an arbitration case?
You owe us nothing. We absorb the filing fees, the time, everything. That's our risk, not yours. We only win when you win.
What if we want to stop working with you?
You can terminate anytime. Cases already in arbitration continue to completion, and you'll still receive those awards. New filings simply stop. No long-term contracts. No traps. This only works if it works for both of us.
Do you use attorneys for this?
Yes. Callagy Recovery is backed by our sister firm, Callagy Law, also founded by Sean Callagy, with 27 years fighting insurance companies. Our attorneys draft every brief. If carriers don't pay, we go after them.
When do I get paid?
First recovery payments typically come in 4-6 months after filing, sometimes up to 9 months. But here's the thing: once that first case resolves, you've been filing new cases every month behind it. So after the initial wait, payments can start flowing in regularly. This can become ongoing revenue, not a one-time thing.
How are you different from other recovery companies?
We're the largest private filer in the US. 90%+ win rate. We consistently recover more per case than other firms. Some charge less, but you often take home more with us because we collect more. The percentage matters less than what lands in your account.
How will I share my cases with you?
After our call, you sign a BAA (HIPAA compliant) so we can review your claims. If it's a fit, we can connect a software to your billing system that automatically sends new cases to us each month. No extra work from your staff after setup. You can review the BAA in advance by clicking here.
Can we track what's been filed, pending, and recovered?
Yes, you'll get full visibility. We provide near-real-time reporting showing every claim in the pipeline: filed, pending, won, paid. Your team can see exactly where the money is at any time. You'll know what's working and what's coming.
I'm not a surgeon. Does this apply to me?
It might. If you're an anesthesiologist, ER physician, or any provider who delivers out-of-network emergency or on-call services, you likely have qualifying claims. Book your free recovery call and we can do a quick review and tell you exactly what qualifies.
I'm a billing manager. Can I bring this to my practice?
Absolutely. Many of our best partnerships start with billing managers who see the underpayments every day and know money is being left on the table. We work directly with you and your team. You're often the hero who brings this to leadership.
Do you partner with RCM or billing companies?
Yes. We handle the arbitration piece that most billing companies don't touch because of the complexity and tight deadlines. You keep doing what you do, we become your arbitration arm. Many RCM companies partner with us and share in the recoveries we generate for their clients.
What about associations, IONM, inadvertent providers?
We work with them all. For associations, we have structures that benefit both your organization and your members. Intraoperative neuromonitoring companies, absolutely - we recover for IONM providers nationwide. Inadvertent provider groups like anesthesia, ER physicians, and others who deliver out-of-network emergency services - yes, this is exactly what we do. If you have out-of-network claims where insurance underpaid, we can help. The process and results are the same; fee structures may vary slightly depending on your setup, but we'll cover all of that on your call.
We're a hospital system. Can you handle that volume?
Yes. We're the largest private filer in the country, processing thousands of arbitrations monthly. We've recovered over $320 million for facilities alone and have dedicated teams for high-volume clients. We scale to fit your size.
Can I refer a colleague who could use this?
Yes. We have a referral program for people who connect us with qualifying providers. If you know surgeons, anaesthesiologists, or practices leaving money on the table, we'd love to talk. Schedule a call with us and we can explain how it works.
